Do you want to improve your health and well being?

Bradford District Care Trust has a free Health Trainer Service to help individuals to achieve their goals for improving their health and well being.

?The Service's Health Trainers support and coach people to set and achieve SMART goals that can help improve physical, mental and social health and well being.  They can help with losing weight, getting fitter, eating healthier, feeling happier and more relaxed.

What do I need to do?
