Preparing for adulthood from the earliest years is covered within Chapter 8 of the new SEND CoP.  We have analysed the Conversion Reviews in Year 9 and Year 11 and the old Learning Difficulties Assessments and believe the current EHCP contents can be improved to make the plan more relevant to young people approaching adulthood. Dr Nazam Hussain (Educational Psychologist), working with colleagues, has developed a new format based on the current EHCP and wishes to consult you on this development before it is introduced in Bradford in September 2015.’
