Schools can make direct referrals now to CAMHS for an autism or ADHD screening.

Direct referrals to the neurodevelopmental pathway can be made using the forms and guidance below

Please note that this may change once the One Trusted Referral Pathway goes live.

The forms below are needed when schools are referring to CAMHS for an autism or ADHD screening.

FORMS

ASD & ADHD observation for school staff

Parents development history

SNAP rating scale

Criteria for an Autism assessment

Referrals accepted for 7 years old upwards.

Traits should be evident at home and at school and be present from an early age.

A SNAP form needed from home, and a separate one from school.

Completed developmental history by parent/s

Completed observation from school

 

Criteria for an ADHD assessment

Referrals accepted from school age.

SNAP form needed from home and one from school.

Completed developmental history by parent/s

Completed observation from school

Evidence of :

Hyperactivity

Impulsivity

Lack of concentration

 

These things should be evident at home and at school and be present from an early age.

 

Forms can be emailed back to CAMH via PMHWNeuroScreening@bdct.nhs.uk or fax-bdct.camhs@bdct.nhs.uk

 

Once received documentation will be reviewed and a letter will confirm whether or not the child has been accepted onto the waiting list for further assessment.
