Joint assessments and Single Education, Health & Care (EHC plan) for children and young people with SEND
 
The group leading this will develop a single EHC plan, which will replace statements of special educational needs. They will also develop a single referral form and one central way to make requests for assessments for an EHC plan.
 
These changes will bring together the work of the children with complex health and disabilities teams, the SEN assessment team, personal
advisors within the education, employment and enterprise team, children’s continuing health care team and the adults’ social care team. This is to create joint working between teams in the support of young people aged birth to 25.
 
The new single referral form and single EHC plan will be tested with young people, parents, carers and services during April and June 2014.
The assessments and EHC plan will be in place for new requests from September 2014. The Government is consulting on the time period for moving existing statements across to the new single EHC plans. This is expected to take place over three years.
